Physician’s Orders for Life-Sustaining Treatment – POLST

Unlike an advance directive (also known as “living will”), a POLST form can save you from treatment during a medical emergency; EMTs can legally follow the wishes stated in a valid POLST.

A POLST must be signed by a healthcare professional to be valid.

The POLST form is intended to be used if a physician would not be surprised if the person died within a year. Get more details about the POLST in the video below.
